How The CoOp collects, uses and protects information submitted through this website.
This policy is a starting template and should be reviewed by counsel before launch, particularly regarding state privacy laws that may apply to your visitors.
What this covers
This policy describes how The CoOp handles information collected through this website, including the ABA Business Assessment and our contact and scheduling forms. It does not cover information handled under a separate written agreement with a member or client organization.
What we collect
Information you provide. Your name, email address, phone number, company name and website, the states you operate in, and the answers you give in the ABA Business Assessment — including business characteristics such as size, payer mix, staffing, financial ranges and operational challenges.
Information collected automatically. Pages viewed, referring source, campaign parameters (UTM tags), approximate location derived from IP address, device and browser type, and interaction events such as starting or completing the assessment.
Information stored on your device. The assessment saves your in-progress answers in your browser's local storage so you can resume. This stays on your device and is cleared when you complete or restart the assessment.
What we do not collect
We do not ask for and do not want protected health information about any individual client, patient or family. Please do not include client-identifying information in any free-text field on this site.
How we use it
- To generate and display your assessment results.
- To email you a copy of your results if you request one.
- To respond to you and, where appropriate, prepare for a conversation you've scheduled.
- To understand which pages and paths are useful, in aggregate.
- To improve the assessment itself.
How we share it
We share information with service providers who operate parts of this site on our behalf — for example our customer relationship management system, email delivery provider, scheduling tool and analytics provider — solely so they can perform those functions for us.
We do not sell your information. We do not share your individual assessment responses with other member companies, and we do not use information provided by one company to advantage another.
We may disclose information if required by law, or to protect the rights, safety or property of The CoOp or others.
How long we keep it
We retain assessment submissions and contact records for as long as needed to support an ongoing or potential relationship, and thereafter as required for legitimate business or legal purposes. You can ask us to delete your information at any time.
Your choices
- Access, correction or deletion. Email us and we'll act on your request.
- Email. Every email we send includes an unsubscribe link.
- Analytics. Our analytics are configured to avoid cross-site tracking; you can also block them with standard browser settings.
Security
We use reasonable administrative and technical safeguards to protect information submitted through this site. No method of transmission or storage is completely secure, and we cannot guarantee absolute security.
Children
This site is intended for business owners and professionals. It is not directed to children and we do not knowingly collect information from them.

We'll post any changes to this policy on this page and update the date below.
